The Occupational Therapy Program at Samuel Merritt University seeks children between the ages of 6 months and 16 years to participate in a free pediatric clinic. Children will receive services from students in the Master of Occupational Therapy Program, whose work is supervised by occupational therapy instructors.
Occupational therapists help children who have difficulty engaging in the typical activities of childhood due to medical conditions, injury, or developmental challenges. An occupational therapist helps a child develop skills to enhance his or her participation in self-care, play and school activities.
Clinic Schedule
The Pediatric Occupational Therapy Clinic at Samuel Merritt University will be held on Monday afternoons for 10 consecutive weeks, starting on Monday, October 8, 2012 and continuing through Monday, December 10, 2012. Clients will be scheduled for a 50 minute therapy session each week. Sessions will start at 1:30, 2:30 or 3:30pm.
How to Enroll
Enrollment in the clinic will begin on June 1, 2012. Children will be enrolled on a first-come, first-served basis.
To begin the enrollment process, please download and print the two forms below. The child's parent or legal guardian should complete the Client Data Form. The child's physician should complete the Physician Order form.

After we receive the completed Client Data Form and Physician Order for your child, we can officially enroll your child in our clinic. Please note that we can only reserve a spot for your child after we receive both of the required enrollment documents. You will receive a confirmation from the Occupational Therapy Department upon enrollment. You will also be provided with an orientation letter along with driving and parking instruction prior to the start of clinic.
