For applicants to Nursing (all programs) and MOT:
Applications should be printed, completed and submitted to the address provided below. All transcripts and letters of recommendation required to review your file should be submitted with the application itself. BSN, ABSN and ELMSN applicants should enclose a copy of the TEAS score report. Please make a copy of your application prior to submission and keep this copy for your records. A $50 non refundable application fee should be made payable to Samuel Merritt University and should be submitted with your application.

APPLICATION FOR READMISSION
If you previously attended Samuel Merritt University as a regularly enrolled student and are seeking readmission to the college, please use this application. Readmission is not guaranteed and applicants must meet current admission requirements and standards. Applications for readmission must be submitted to the Office of Admission a minimum of 60 days prior to the beginning of the term for which readmission is being requested.
